- Medically Necessary: Covered in full for in-network and up to $210 reimbursement for out-of-network
- Elective Conventional: $150 allowance + 15% off any remaining balance for in-network and up to $105 reimbursement for out-of-network
- Elective Disposable: $150 allowance for in-network and up to $105 reimbursement for out-of-network
Any claims regarding Safety Glasses go through the HSE department and do not go through our Vision Insurance.
