Qualifying Life Events

What is a Qualifying Life Event?

Outside of your enrollment period, you may not make any changes to your plans without a Qualifying Life Event, which grants you a special enrollment period. When you experience a QLE, you have 30 days to notify Benefits that you would like to make changes to your benefits.

The life event date is the baby’s date of birth.

  • Required documentation:
    • Proof of birth
    • Legal adoption paperwork
    • Court documents outlining the permanent guardianship

The life event date is the date you were married.

  • Required documentation:
    • Marriage certificate

The life event date is the date the divorce or annulment was finalized.

  • Required documentation:
    • Final divorce decree
    • Final decree of annulment

The life event date is the first day you no longer have coverage.

  • Required documentation:
    • Letter confirming the loss of coverage for those being enrolled
    • For dependents not previously enrolled in Liberty coverage, proof of legal relationship:
      • Marriage certificate or common law affidavit (if recognized by state law) for a spouse
      • Birth certificate(s) for children or stepchildren

The life event date is the day coverage under the new plan becomes effective.

  • Required documentation:
    • Letter confirming the effective date of the new coverage and which dependents are enrolled

Submitting a Qualifying Life Event

If you experience one of the qualifying life events, you MUST submit the event and supporting documentation to the Benefits team within 30 days. For help with submitting the event, toggle through the following steps:

  1. Go to the “My” tab
  2. Click the “Benefits” tile
  3. Select the life event reason from the options and enter the event date.
  • Follow the prompts to make your desired changes. You will need to click through each benefit you wish to make changes to.
  • Do a final review and select “Checkout.”
  • Upload the corresponding documents into the system for proof of the life event.
  • For a spouse not previously enrolled, you will need to provide a marriage certificate. For children not previously enrolled, you will need to provide their birth certificate(s).

Important Reminders

  • You will need to review every benefit before you can check out.

  • If you are adding a dependent, you will need to click through each benefit you wish to add them to, check the box next to their name, then “Update Cart.” The system will not automatically add them to your current benefits.

  • If you are removing coverage for a dependent, you will need to click through each benefit you wish to remove them from, uncheck the box next to their name, then “Update Cart.” The system will not automatically remove them unless you remove them as a family member under “My Family.”

  • Once you’ve finalized your changes, select “Review and Checkout.” Complete one final review of your changes, then select “Checkout” if everything looks good!

  • The Benefits Team cannot review a life event without supporting documentation. Please either upload it or email it to benefits@libertyenergy.com.

  • Life events may come with a need to update your beneficiaries. Don’t forget to update them for your life insurance and log in to Principal to make the update for your 401(k)!
